Neither gap is new information. Configuration validation was scoped as ticket PLAT-892 fourteen months before this incident, but was deprioritized twice in planning because it was judged low-risk toil. This is also the second config-caused outage this quarter: INC-4308 in April was a similar YAML mistake, that time in the rate-limiter configuration, producing a comparable failure mode. The missing safeguard is not an isolated oversight — it is a known, accepted risk that has now caused two SEV-level incidents in the same quarter.
